Mission
To improve the cardiovascular health of the residents of
Jacksonville through more effective citywide prevention, detection,
and treatment of heart attacks and strokes, and their risk factors.
Vision
To improve the quality of life for all people in Jacksonville
by: Reducing the morbidity and premature mortality due to heart
diseases and stroke, and by eliminating cardiovascular health
disparities.

Heart Disease
1. Reduce coronary heart disease deaths.
Year 2010 Target: 166 deaths per
100,000 population.
Year 1999: 224.5 deaths per
100,000 population.
Year 2000: 212.7 deaths per
100,000 population.
2. Increase the proportion of adults
aged 20 years and older who are aware of the early warning signs
and symptoms of a heart attack and the importance of accessing
rapid emergency care by calling 911.
3. Increase the proportion of eligible patients with heart attacks who
receive artery-opening therapy within an hour of symptom onset.
4. Increase the proportion of adults aged 20 and older who call 911
and administer cardiopulmonary resuscitation (CPR) when they
witness an out-of-hospital cardiac arrest.
5. Increase the proportion of eligible persons with witnessed
out-of-hospital cardiac arrest who receive their first
therapeutic electrical shock within 6 minutes after collapse
recognition.
6. Reduce hospitalizations of older adults with congestive heart
failure as the principal diagnosis.

Blood Pressure

10. Increase the proportion the proportion of adults with high
blood pressure whose blood pressure is under control.
Year 2010 Target: 50%.
11. Increase the proportion of adults with high blood pressure who are
taking action (for example, losing weight, increasing physical
activity, or reducing sodium intake) to help control their blood
pressure.
Year 2010 Target: 95%.
12. Increase the proportion of adults who have had their blood
pressure measured within the preceding 2 years and can state
whether their blood pressure was normal or high.

Cholesterol
13. Reduce the mean total blood cholesterol levels among
adults.
Year 2010 Target: 199 mg/dL
